Lost in Transition? New edition of CHILD's newsletter

The topic of this issue of CHILD's Newsletter is 'Lost in Transition?' Several of CHILD's researchers share their results of what happens and what can happen if transitions are poorly done in medical care when entering the adult health care coming from child health care.
